MONTHLY STATUS REPORT INSTRUCTIONS This is a guideline for producing your Monthly Status Report. The Status Report must be turned in to gain admission to the ASI Seminar. Please place your Name and Age at the top of the report. What follows are intended as recommendations and not requirements. Presentation guidelines: Typed, double spaced, if more than one page, staple the pages together. The Report should be brief and succinct. Place the Report in something that does not seal closed, such as a two pocket folder. The seven areas in which you can report with sample comments and answers. Please comment on the areas which apply to you. Write the number and the name of the area in which you are reporting and comment on all the guidelines (sub-points) presented in each area below. If the area does not apply, place the number and name of the area with 2 blank spaces below it (this will maintain a consistent numbering sequence each month.) 1. BUDDHIST PRACTICE Meditation - morning recommended time 60 min. and evening (before sleeping) recommended time 30 min. Mindfulness. Interesting experiences or realizations. Books read. Example: I meditated every morning for one hour. I meditated every evening for 30 minutes. (Mindfulness will be explained by Rama at the Dinners) Generally was more relaxed and still this month. I noticed my dreams improved dramatically in the last 2 weeks. They are much higher and more positive. I re-read the 'Power of Silence' and 'Tantric Mysticism of Tibet'. 2. ASI UPDATE A. PERSONAL CAREER State where you are currently working and how much money you are making per hour, or if you are an employee, per year. State a realistic goal for your next job and note when you anticipate looking for a new one. Example: A. I currently have a contact at Fraznoid Brothers, Inc. at 62.50 per hour doing Xwindows Programming. My contract ends in January 1992 and I am currently looking for another contract with more responsibility such as project leader to up my rate to 75.00 + per hour. B. ASI AI Clips Classes and Comments. Clips home work (reading, practice on computer). AI Books, manuals and magazines read. Example: B. I attended all four nights of Clips Classes and did the homework assignment for last month. I admired Koan's dressing style and hope to emulate a similar style in the near future. I spent 30 minutes each night for 2 weeks reading the Clips manuals and developing the application for our homework assignment. I read this months issue of AI Expert and the first 2 chapters in 'A Crash Course in Artificial Intelligence and Expert Systems'. C. BUSINESS Recommendations regarding your career and/or business are, of course, optional and should be pursued at your own discretion. Business Plan - (Resubmit only when it changes). Sample marketing materials (Resubmit only when it changes) IE: Cards, Brochures, Corporate Image (stationary, etc.) Business Update - Contracts your company currently holds, companies you are in negotiation with (gave one or more presentations and they are seriously interested in your product or services), the number of presentations you did during the month. If you have a product (not your Infinity products) state your sales. Example: C. (Here you should submit as a separate document, your business plan. You can follow the guidelines for a business plan given in the Successful Corporations Manual from our class this summer or summarize your plans in the following way: A.) What types of services your company provides. B.) What sectors of the industry or business will you service. C.) Marketing plan (how will you sell your services). D.) Describe the where, what, when and how of operating your business. E.) Financial goals and strategies for obtaining your goals. F.) Research and development for products (if this applies).) Enclosed are my current marketing materials as well as my as well as my Corporate Image. CURRENT CONTRACTS: Action Videos, Inc. Currently developing a PC application to track customers and rentals and keep inventory of their video library. Value of contract 5,000. New York Health and Racket Clubs,Inc. Currently performing systems integration work to tie a new fitness assessment package into their customer databases for their lO clubs throughout the city. Value of contract 10,000. PROSPECTIVE CLIENTS: UPS - working on providing them 2 OS/2, Sequel Server programmers at 70.00 per hour. Merril Lynch - Presented our system integration services to assist them a large project which entails getting their mainframe customer tracking system to talk to PC workstation customer applications and vice versa. Estimated value 40,000. 3. MARTIAL ARTS AND PHYSICAL DEVELOPMENT All of the recommendations regarding physical fitness are, of course, optional and should be consistent with your own ideas about physical fitness and well being. A. State your Height and Weight. Examine your physical fitness by measuring your strength, flexibility and endurance levels. Recommended exercises (if you are doing any of these exercise, state what your are currently doing.) STRENGTH: Pushups, Pullups, Situps, Weight training, etc. FLEXIBILITY: Yoga, Gymnastics or Gymnastic type exercises. ENDURANCE: Swimming, Running, Aerobics, etc. When Combined, these exercises will give you physical health and energy. Example: A. I am 5'9" and weight 135 lbs. STRENGTH: I am currently doing 20 pushups, 5 Pullups, 40 Situps every other day. I life weights 3 days per week at the gym (5lb weights). ENDURANCE: I do 30 minutes of aerobics three times a week and run 3 miles every weekend. FLEXIBILITY: I do yoga (10 postures) 45 minutes, every other day. My weekly physical routine consists of going to the gym 3 nights per week (my workout routine also assists in developing strength and endurance (Approx. 1.5 hours)) B. State your goals for improving in each of the three areas. Find an ideal weight for yourself that is realistic. gradually work your way to achieving that weight as well as improving in each of the three areas. Set realistic goals (for next month) for developing strength, flexibility and endurance (for example increase reps, miles, etc. in small increments, don't set a goal to go from 50 situps to 100 in one month). If you are considering a physical program, make sure to get direction or a physical exam to assess your current level and design an appropriate program for your level and goals. Start any new program moderately as to not injure. (As indicated in the ASI registration/release form, by engaging in a physical development program that you design, you are acknowledging that you have regular physical checkups and moderate any physical activity consistently with your physician or other health practitioner.) Example: B. STRENGTH: to increase my pushups to 25, pullups to 7 and situps to 45; to increase the weight adjustments I can handle at the gym by 5 pounds. FLEXIBILITY: to gain balance and more flexibility in my yoga postures. ENDURANCE: to extend my aerobics time to 40 minute sessions; to run 3 1/4 miles a week. C. Martial Arts Classes taken - How often do you go? State the style of martial arts you are taking, your current ranking or level, the location of your dojo, the name of your sensei, when do you test next, tournaments you participated in. Example: C. I have a green belt in Judo. I study at the New York Judo Institute with Sensei Woo. My next belt test is in 2 weeks. I will be fighting in a tournament at the end of the month. I attended 8 martial arts classes this month. 4. ASI PACKAGES Title of your package and the team members names. What you did this month. Next months goals. Example: Frogmania - Team: Jasper Green, Bob Brown and Garuda Samir. We finish prototyping 3 mazes and now have a working user interface. Our goal for next month is to develop 3 more maze prototypes and enhance the user interface and create an online help facility. 5. YOUR APPRENTICE ACTIVITY UPDATE With the understanding that your are entirely responsible for your interactions with any individuals you choose to teach, Dr. Lenz and ASI would like a monthly summary or your general attempts to local appropriate students as well as a monthly update on whether you will be bringing one or more of them to dinner the next month (starting in January, note: Dinner reservations must be made in advance by calling the reservation phone number). Example: (These are not guidelines as to how you should locate or deal with potential students of yours, it is merely an example.) I created a poster for a lecture series I will be giving in the next 3 weeks. i will give 3 lectures, one per week for 3 weeks at Taft College on the subject of meditation and psychic development in the 1990s and how it can benefit you in your career. I have found what I feel is an excellent first candidate and will be bringing her to the next dinner. 6. INFINITY PLUS CONSULTING PROGRAM If your product has been approved verbally by Dr. Lenz, give a brief description of the product. In what state of development is your product? What are your goals for next month? How did you do on meeting your goals for last month? Example: I am developing a report write tool which works with Sybase. I turned in my formal product outline/description at the last Infinity dinner. I am in the testing phase of the product development life cycle. the product is in pre-beta form. My goals for next month is to get the product in to beta form and have the major bugs worked out. I met my goal for last month by completing a rough of the product. 7. BLACK BELTS Give a detailed discussion of you martial arts activities. State what your next belt will be. Special Training, Tournament activities. Example: Sorry Black Belts, your on your own for the discussion of your martial arts activities! I will be attending a special training seminar in Colorado in 3 weeks. I fought in 2 tournaments this month and won one fight and ranked second place in my division.
